The Fair Credit Reporting Act (FCRA), as amended, requires that we advise you that, for purposes of employment only, a consumer report may be made that may include information about your character, general reputation, personal characteristics, attendance records, disciplinary reports, letters of reprimand, criminal records, and other disciplinary records. Upon written request, additional information as to the nature and scope of the report, if one is made, will be provided, in the event the report contains information regarding your character, general reputation, personal characteristics, attendance records, disciplinary reports, letters of reprimand, criminal records, and other disciplinary records.
AUTHORIZATION AND RELEASE
During the application process, and at any time during my subsequent employment, I hereby authorize MEMBERS HERITAGE FEDERAL CREDIT UNION or its agents to procure a consumer report that I understand may include information regarding my character, general reputation, personal characteristics, attendance records, disciplinary reports, letters of reprimand, criminal records, and other disciplinary records. The report may be compiled with information from credit bureaus, court record repositories, departments of motor vehicles, past or present employers, educational institutions, governmental occupational licensing or registration entities, business or personal references, or any other sources from which information is obtained. I understand that I may request a complete and accurate disclosure of the nature and scope of this consumer report, to the extent that same includes information bearing on my character, general reputations, personal characteristics, attendance records, disciplinary reports, letters of reprimand, criminal records, and other disciplinary records.
In consideration of the acceptance of my application and/or my continued employment by MEMBERS HERITAGE FEDERAL CREDIT UNION, I release MEMBERS HERITAGE FEDERAL CREDIT UNION and its agents, credit bureaus, court record repositories, departments of motor vehicles, past or present employers and educational institutions, governmental occupational licensing or registration entities, business or personal references, and any other source(s) of information contained in the consumer report from any and all liability arising out of any response or disclosure made in connection with a request from MEMBERS HERITAGE FEDERAL CREDIT UNION for a consumer report, and/or any personnel action taken on the basis of such a response or disclosure.
